CSM Lafayette A. Cook Jr.

Command Sergeant Major Cook hails from Savannah, Georgia where he entered the Army in November 1987. He attended Basic Training and advanced individual training at Fort Sill, Ok and was awarded MOS 13B, Field Artillery Cannon Crew Member. After severing ten years in Field Artillery he reclassified to Air Defense Artillery as a 14E, PATRIOT System Mechanic, completing advanced individual training at Fort Bliss, TX.

His duty stations have included 3rd Battalion, 22D FA Fort Sill, OK, 1st Battalion FA Fort Lewis, WA, 27th USAFAD Turkey, 260th FA Det Fort Rucker, AL, 1ST Battalion 84TH FA HI, 2/319th AFAR Fort Bragg NC, 1-6 ADA (THAAD) Fort Bliss TX, 6TH BDE ADA Fort Bliss TX, C Battery 1/43 ADA, Osan AB Korea, C 3/43 ADA Fort Bliss, TX and 32d Army Air Missile Defense Command (AAMDC), 5th BN 5th ADA Joint Base Lewis MacCord (JBLM).

Command Sergeant Major Cook has served in a variety of positions throughout his Army career to include Section Chief; Squad Leader; Senior Missile Handler; Maintenance Platoon Sergeant; Fire Control Platoon Sergeant; Senior Master Evaluator; First Sergeant; Operation Sergeant; and Operation Sergeant Major 5th BN 5th ADA (SHORAD).

His military and civilian education includes all levels of the Noncommissioned Officer Education system, as well as Basic Instructor Training Course, Small Group Instructor trainer Course, coveted Airborne and Air Assault School, Nuclear Cannon Assembly, and Equal Opportunity representative. He has also obtained a Bachelor’s Degree in Human Resources Management from Park University.

Command Sergeant Major Cook’s awards and decorations include Joint Meritorious Unit Award, Army superior Unit award, Meritorious Service medal (four Oak Leaf Cluster), Army commendation Medal, with (four Oak Leaf Clusters), Army Achievement Medal, with (two Oak Leaf Clusters), Good Conduct Medal (eight award), National Defense Service Medal (Second award), Southwest Asia Service Medal (three Bronze stars), Korean Defense Service Medal, Global War on Terror Service Medal, Humanitarian Service Medal, NCO Professional Develop Ribbon with (numerical 4), Army Service Ribbon, Overseas Service Ribbon with (numerical 4), Saudi Arabia Liberation Medal, Kuwait Liberation Medal and the Drivers Badge. He is a recipient of the Honorable Order of Saint Barbara.
