Social Security Card or a Letter from the Social Security Administration,
(letter must have name and Social Security Number on it) or W-2 Form.
Department of Veterans Affairs (DVA) Letter stating Honorably Discharged Veteran rated
100% (not entitled to retired pay) by the DVA. DVA letter MUST certify that the Veteran is
entitled to Commissary and Exchange privileges and indicate whether a medical re-evaluation
is necessary. (Initial card only.)
DD Form 214 reflecting Honorable Discharge. (Initial card only.)
NOTE: Dependents MUST already be established as
dependents on the VA record. They will receive medical benefits through
the Department of Veterans Affairs. For documents required for dependent
